This form is provided to facilitate the verification of your secondary academic records by WES whereas ETX.NG only delivers the completed forms electronically to WES saving you time and money. Please note that you are responsible for contacting the appropriate examination board to obtain the required information.

1. Complete this form using the PIN code and serial number found on the scratch card or the electronic PIN.

2. You must include your WES reference number.

3. Complete the required information to generate "Self Delivery Request"

Once completed and you have obtained a reference number, you will be required to send us a copy of your certificate to validate the form information you provided before we can deliver directly to WES. Kindly log into the self-service and upload a copy of your certificate.
